The Activation Report (PC61) is a comprehensive report designed to provide detailed information about customers who were activated within a specified date range.
This report can be customized to focus on a specific agent or encompass all agents within the company.
It can be generated based on either the order date (when the order was created) or the activation date (when the customer was activated in Telgoo5).
The user can generate report based for specific enrollment type which can be ACP,Lifeline, or Prepaid.
Based on this report, users can generate detailed information on customers, including the agent who enrolled them, the user's chain, the enrollment source, and the account status. Additionally, the report provides additional fields covering plan details, device information, and carrier specifics.

Activation report fields definitions:

- CREATED DATE: The date when the customer was created in Telgoo5
- ORDER DATE: The date when the order was placed
- ACTIVATION DATE: The date when the customer was activated in Telgoo5
- CUSTOMER ID: An auto-generated customer id assigned to the customer when they are created in Telgoo5
- ENROLLMENT ID: An auto-generated enrollment id assigned to customers as soon as they begin the enrollment process in Telgoo5 and the id used to send requests to NLAD
- FIRST NAME: The first name of the customer
- LAST NAME: The last name of the customer
- ADDRESS1: The service address of the customer
- ADDRESS2: The service address line 2 of the customer
- CITY: The city of the service address of the customer
- STATE: The state of the service address of the customer
- ZIP: The zip code of the service address of the customer
- SSN: The last four digits of the social security number of the customer
- DOB: The date of birth of the customer
- IS HOUSEHOLD: Determines if the customer has multiple households at the service address
- IS TRIBAL: This field shows if the customer's address falls on tribal lands or not
- IS SHELTER: Determines if the customer's address is a shelter address
- IS STATE DB: Determines if the customer was verified through any of the state databases for eligibility for lifeline or ACP service.
- SIM: The SIM number assigned to the customer for activation
- MDN: The telephone number assigned to the customer
- EMAIL: The customer's email address at which all the email communications will be sent
- ALTERNATE PHONE NUMBER: The customer's alternate number at which they can be contacted
- DEVICE ID: The IMEI of the phone or tablet assigned to the customer
- MAKE MODEL: The model description of the device offered
- NLAD SUBSCRIBER ID: The subscriber id generated upon successful enrollment in NLAD for ACP or lifeline orders.

- PLAN NAME: The plan which was offered and selected by the customer
- PRODUCT TYPE: Shows the product received by the customer, such as Freephone, Tablet, etc.
- CARRIER: The network carrier on which the customer was activated.
- TABLET SUBSIDY QUALIFICATION
- QUALIFYING PROGRAM: The program on the basis of which the customer was deemed eligible for ACP or lifeline services, such as SNAP, SSI, etc.
- SOURCE: The source of the order, such as website or agent, through which the customer placed the order.
- AUTHORIZE CODE:
- MASTER AGENT: The master agent of the agent who enrolled the customer
- DISTRIBUTOR AGENT: The distributor of the agent who enrolled the customer
- RETAILER AGENT: The retailer of the agent who enrolled the customer
- AGENT: The agent/employee who enrolled the customer
- DL NUMBER: The driving license number of the customer
- WIN BACK: If the customer was disconnected and then reconnected back, this field shows if they were won back after disconnection
- SPONSOR ID: The sponsor id or affiliate number used, if applicable
- CURRENT APS: The APS on which the customer's service is activated.
- IMPORTED ORDER ID: If the customer was imported to Telgoo5 from any other system which MVNO used prior to Telgoo5, this is the order id of the customer in the previous billing system
- IMPORTED ACCOUNT NO: If the customer was imported to Telgoo5 from any other system which MVNO used prior to Telgoo5, this is the account number of the customer in the previous billing system
- ACCOUNT STATUS: The current account status of the customer in Telgoo5
- CURRENT APS: If you are using PLUM as your carrier, this shows the plan Aps of the customer
- PORT-IN STATUS: This shows if customer ported their number
- AGENT LOGIN ID: This is the login id of the agent who enrolled the customer
- MEDICAID ID: Medicaid details if used for eligibility check
- BQP FIRST NAME: This is the qualifying beneficiary first name
- BQP LAST NAME: This the qualifying beneficiary last name
- BQP BIRTH DATE: The is the date of birth of the qualifying beneficiary
- NLAD ENROLLMENT TYPE: This field shows if the customer was enrolled or transferred from other ETC
- ENROLLMENT TYPE: This shows if the order is a Lifeline order, ACP or prepaid order
- CONSENT FORM AVAILABLE(Y/N): If customer provided a consent form for ACP services during the sign up process this will be Y
- ID PROOF AVAILABLE(Y/N): If ID proofs were collected by Telgoo5 for the customer during sign up process this will be Y
- DEVICE REIMBURSEMENT DATE: This field displays the date on which the device reimbursement will be provided for ACP orders.
- DEVICE EXPECTED RATE: For ACP orders, this field shows the amount that the MVNO will receive from USAC as reimbursement
- DISCONNECT REASON: If customer has been disconnected this shows the reason for disconnection
- DEACTIVATION DATE: This is the disconnection date of the customer
- REVIEWED BY: This field displays the name of the agent or source who reviewed the customer application.
- SAME MONTH DISCONNECTION: This field indicates whether the customer disconnected in the same month in which they were activated. If yes, it will display as "Yes".
- ACTIVE PERIOD: If the customer is disconnected, this field shows the number of days the customer was active in the system before deactivation.
